Job Description
- The successful candidate will work closely with the multidisciplinary healthcare team to provide palliative care to clients, ensuring they receive the medical care required to remain at home.
- You will be responsible for making home visits to clients in designated geographic territories, performing assigned duties, and monitoring clients' conditions.
- You will need to follow up with, execute, and document doctors' orders, perform client assessments, and maintain accurate records.
Qualifications
- A current license as a Registered Nurse (RN) in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience is strongly preferred, but not required.
- Solid computer skills, with experience using electronic medical records (EMR), are preferred.
- Ability to travel to cases as assigned.
