As a Hospice RN, you will provide medical care and support to clients in their homes, working collaboratively with other healthcare professionals to deliver high-quality patient care and support.

Responsibilities
- Making home visits to clients in designated geographic territories.
- Providing medical care and support to clients, including administration of medication, wound care, treatments, and procedures, and comfort care.
- Collaborating with other healthcare professionals to develop and implement personalized care plans for clients.
- Documenting client information and progress in accordance with company policies and procedures.
Qualifications
- A current license as a Registered Nurse (RN) in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred, but not required.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
