Job Description
The Program Specialist will play a vital role in our Habilitation program, working closely with the Clinical Manager to act as internal case manager and advocate for clients.
The successful candidate will participate in Person-Centered Planning processes, collaborate with clients, families, Case Managers, Hab Techs, and other teams to implement and evaluate client care needs and individual goals.
The Program Specialist will make regular client visits to evaluate progress and monitor that client services are implemented according to the Person Centered/Individual Support Plan.
