Healthcare Professional - Home Visits

As a Physical Rehabilitation Specialist, you will be working with a diverse group of clients, each with their own unique needs and challenges. You will work closely with other healthcare professionals to develop and implement individualized care plans that meet the specific needs of each client.

The responsibilities of this role include:

• Making home visits to clients in designated geographic territories.
• Performing diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, patterns of ambulation, strength, endurance of muscles, balance testing, and safety assessments.
• Implementing integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Developing and implementing appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate.
• Educating and instructing clients, family members, or other client representatives in rehabilitative care and activities necessary to promote health, safety, and independent living.
