This position involves making 10-15 visits per day on weekends, performing SOC Admissions for our valued clients. While prior home healthcare experience and knowledge of Oasis/HomeCare HomeBase EMR are beneficial, they are not essential.
Responsibilities:
- Create customized care plans tailored to patients' needs
- Establish effective communication channels with Clinical or Client Services Manager for prompt updates
- Deliver medication, wound care, treatments, and procedures as dictated by patient requirements
- Monitor patient progress and notify Clinical or Client Services Manager of any significant changes
- Accurate execution and documentation of doctors' orders
- Conduct patient assessments when necessary
- Coordinate case management efforts
- Document observations, interventions, and evaluations related to patient care and services utilizing state-of-the-art tablets
Necessary Qualifications for the Registered Nurse (RN):
- A valid license as a Registered Nurse in Maryland
- Prior home care experience preferably with Oasis/HomeCare HomeBase EMR
- Degree from an accredited nursing program
- Self-motivation and strong organizational skills
- Excellent interpersonal and communication abilities
- Familiarity with electronic medical records (EMR)
- Ability to travel to assigned locations
