About the Opportunity
As a home health care LPN, you will be an essential part of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care needed to stay at home. Your responsibilities will include making home visits to clients living in designated areas, performing assigned tasks such as administering medication, wound care, treatments, and procedures.
You will monitor clients' conditions, report any changes to the Clinical Manager and Client Services Manager, and follow up with doctor's orders. You will also conduct client assessments as necessary, manage cases, and coordinate services. Accurate documentation of observations, interventions, and evaluations related to client care management and services provided using electronic medical records on a state-of-the-art touch pad tablet is crucial.
Key Responsibilities
- Making home visits to clients residing in designated areas.
- Performing assigned tasks, including medication administration, wound care, treatments, and procedures.
- Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
- Following up with, executing, and documenting doctor's orders accurately.
- Conducting client assessments as necessary.
- Managing cases and coordinating services.
Licensed Practical Nurse Qualifications:
- A current license as a Licensed Practice Nurse in the state of Nevada.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Prior home care experience strongly preferred but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
