About the Role
This is an excellent opportunity for a compassionate and dedicated Registered Nurse (RN) to join our team in providing palliative care to clients in their homes. As a hospice nurse, you will be an integral member of a multi-disciplinary health care team that provides quality care to clients in need.
Key Responsibilities
- Develop and implement individualized care plans that meet the unique needs of each client
- Provide high-quality nursing care to clients in their homes, including administration of medication, wound care, treatments, and procedures
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication
- Document all interactions with clients and family members, including progress notes, medication administration records, and other relevant information
- Participate in ongoing education and training to stay current on best practices and industry developments
Requirements
- A current license as a Registered Nurse in NJ.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
