This is a full-time opportunity for a licensed Physical Therapist to make a meaningful difference in the lives of seniors by providing personalized Physical Therapy services tailored to their unique needs.
Responsibilities
- Visit clients in designated geographic territories, delivering patient-centered care and utilizing evidence-based practices to optimize client outcomes.
- Conduct thorough assessments, including diagnostic tests and measurements, to inform the development of individualized care plans.
- Implement care plans through manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and quality of life.
- Collaborate with nursing and other interdisciplinary team members to revise and update care plans as needed.
- Educate and instruct clients, family members, or representatives on essential aspects of rehabilitative care and self-care strategies.
- Maintain accurate and detailed documentation of client interactions, treatments, and progress, using a state-of-the-art touch pad tablet.
Requirements
- A valid Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Completion of a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as verified by school transcript or diploma.
- Excellent communication, problem-solving, and time management skills.
- Proficiency in electronic medical records (EMR) systems, with preference given to candidates with prior experience.
