BAYADA Home Health Care is seeking a skilled and compassionate Home Health Occupational Therapist to join our team in Seabrook, New Hampshire.
Job Description
We are currently looking for an experienced Occupational Therapist to work on a part-time or per diem basis in our New Hampshire office, located in Nashua. The successful candidate will perform home health visits for adult and geriatric patients throughout Portsmouth, Hampton, and Seabrook.
The Occupational Therapist will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients. This role involves:
- Performing thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Assessing cognitive, psychological, and motor aspects of performance and developing individualized intervention plans to achieve favorable outcomes.
- Identifying equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
- Continually assessing and revising the occupational therapy care plan and participating with nursing in the multidisciplinary care plan as appropriate.
- Educating and instructing patients, family members, or other patient representatives in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Requirements
To be considered for this role, you must have:
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
