We are currently looking for an experienced Outpatient Occupational Therapist, OT, for full-time or part-time opportunities performing outpatient therapy in assisted living communities for our Senior Living office, located in Blue Bell, PA.
This opportunity allows you to spend more one-on-one time with your patients and focus on quality care. As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Responsibilities:
- Evaluate patients in designated senior living communities or group homes to develop personalized plans of care.
- Operate at the top of licensure utilizing current research-based approaches to maximize functional independence for clients.
- Work closely with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
- Educate clients, families, and caregivers on care plans to achieve a team approach to maximizing functional independence.
- Perform evaluations, treatments, and discharge planning for clients using an outpatient EMR platform.
Qualifications:
- A current PA Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills, prior experience with electronic medical records (EMR) preferred.
