We are looking for an experienced Occupational Therapist who will provide high-quality rehabilitative care to adult and geriatric clients in their homes. The successful candidate will be an integral member of a multi-disciplinary health care team that provides patient-centered care.
Duties and Responsibilities:
- Evaluate patients' physical and cognitive abilities to develop personalized treatment plans.
- Perform outpatient therapy evaluations and assessments to identify areas for improvement.
- Collaborate with healthcare professionals to ensure seamless transitions of care.
- Provide education and training to patients, family members, and caregivers on proper techniques and strategies for maximizing functional independence.
- Document patient progress and outcomes using electronic medical records (EMR) platforms.
Requirements:
- A current PA Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills; prior experience with EMR platforms preferred.
