Job Description:
BAYADA Home Health Care is seeking an experienced Occupational Therapist for a part-time opportunity in Northeast Ohio. As a home health Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients.
Main Responsibilities:
- Perform outpatient therapy evaluations and develop plans of care for patients in designated senior living communities or group home settings.
- Work closely with home health therapy partners to transition care from home health to outpatient services.
- Educate clients, families, and caregivers on care plans to achieve functional independence.
Requirements:
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
