Job Title: Home Health Occupational Therapist
BAYADA Home Health Care seeks an experienced Occupational Therapist for a part-time opportunity performing outpatient home health visits in Northeast Ohio.
Responsibilities:
- Perform outpatient therapy evaluations to develop plans of care for patients in senior living communities or group homes for adults with intellectual disabilities.
- Work with home health therapy partners to transition care from home health to outpatient services and achieve client-centered goals in their living environment.
- Educate clients, family, and caregivers on care plans to maximize functional independence.
Requirements:
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
