Job Title: Home Health Occupational Therapist

BAYADA Home Health Care is seeking a compassionate and skilled Occupational Therapist to join our team in Holmdel, New Jersey.

Job Summary:

We are looking for an experienced Occupational Therapist to provide outpatient home health visits to adult and geriatric patients in Independent and Assisted Living Communities throughout Monmouth County. As a member of our multidisciplinary healthcare team, you will play a vital role in delivering high-quality rehabilitative care and skilled nursing services to clients in the comfort of their own homes.

Responsibilities:

• Perform comprehensive therapy evaluations to develop individualized plans of care for patients.
• Utilize current research-based approaches to maximize functional independence for clients.
• Work closely with home health therapy partners to transition care from home health to outpatient services.
• Educate clients, family members, and caregivers on care plans to achieve a team approach to maximizing functional independence.

Qualifications:

• A current NJ Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
