About the Role
As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing services to clients. Your responsibilities will include:
- Making home visits to clients in designated geographic territories to assess their needs and develop personalized care plans.
- Performing diagnostic tests and measurements to evaluate the client's mobility, strength, balance, and overall physical function.
- Implementing evidence-based interventions to promote wound healing, prevent further injury, and enhance the client's quality of life.
- Educating clients and their family members on how to safely perform daily activities, manage pain, and maintain their independence.
- Documenting observations, interventions, and evaluations in a timely and accurate manner using a state-of-the-art touch pad tablet.
Qualifications and Skills
- A current Maryland Physical Therapist - PT license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently, manage time effectively, and prioritize tasks to meet deadlines.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience using electronic medical records (EMR) preferred.
