Job Summary
The role of the Home Health Physical Therapist involves providing rehabilitative care and skilled nursing services to clients in their homes, enabling them to receive medical treatment required to remain at home. This position requires a highly skilled professional who can work independently and effectively manage time.
Key Responsibilities:
- Client Visits: Conduct home visits to clients within designated geographic areas.
- Clinical Assessments: Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing Care Plans: Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions tailored to each client's needs.
- Ongoing Assessment and Revision: Continually assess and revise the Physical Therapy - PT care plan, collaborating with nursing on the multidisciplinary care plan as necessary.
- Educate and instruct clients, family members, or other representatives on rehabilitative care and activities promoting health, safety, and independent living.
- Documentation: Accurately document observations, interventions, and evaluations related to client care management and services provided using state-of-the-art touch pad tablets.
