Role Summary
This role requires a licensed Physical Therapist with one year of prior clinical experience to join our team as a Home Health Rehabilitation Therapist. The ideal candidate will have excellent assessment and intervention skills, as well as strong communication and documentation abilities.
Key Responsibilities
- Perform diagnostic tests and measurements to assess clients' physical abilities.
- Develop and implement individualized care plans that address clients' specific needs and goals.
- Collaborate with nursing and other healthcare professionals to ensure comprehensive care.
- Provide education and training to clients and their families on rehabilitative care techniques.
Requirements
- A current PA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
