BAYADA Home Health Care is seeking a highly skilled Physical Therapist to join our team in Fort Collins, Colorado. As an outpatient Physical Therapist, you will be an integral member of a multi-disciplinary healthcare continuum team that provides high-quality services to clients affording them the opportunity to age in place.
Responsibilities:
- Make outpatient visits to clients in designated senior living communities in the Loveland area.
- Supervise Physical Therapy Assistants as necessary or applicable, in accordance with state licensure requirements and professional standards.
- Attend and contribute to patient care, staffing conferences, and other related meetings.
- Make referrals to other disciplines to address comprehensive client needs.
Qualifications:
- A current CO Physical Therapist license.
- Outpatient experience preferred but not required.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Dynamic interpersonal, organizational, and problem-solving skills.
