About the Role: BAYADA Home Health Care is seeking a highly skilled Physical Therapist to join our team in providing home-based care services. As a member of our multidisciplinary team, you will work closely with clients, family members, and healthcare professionals to deliver high-quality rehabilitative care.
Responsibilities:
- Conduct home visits to assess client needs and develop personalized care plans.
- Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance therapy.
- Collaborate with nursing staff to ensure seamless care coordination.
- Educate clients and their families on self-care techniques and healthy living strategies.
- Document all interactions and interventions in our state-of-the-art electronic medical record system.
Requirements:
- Current licensure as a Physical Therapist in New Jersey.
- Minimum one year of recent experience in physical therapy, preferably in home health or geriatrics.
- Strong interpersonal and communication skills, with ability to work independently and manage time effectively.
- Proficiency in using electronic medical records and other software applications.
- Familiarity with Medicare, Medicaid, and other payor sources a plus.
