About the Role

Key Responsibilities
Conduct initial assessments and evaluations of clients to identify their unique needs and develop personalized treatment plans
Deliver high-quality physical therapy sessions to clients in their homes, adapting to their individual needs and abilities
Monitor client progress, adjusting treatment plans as needed to ensure optimal outcomes
Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care and support
Maintain accurate records and documentation of client progress and treatment plans
Requirements
A current Physical Therapist license in Hawaii
A minimum of one year of recent work experience as a Physical Therapist
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
Solid computer skills and prior experience with electronic medical records (EMR)
