Job Description
BAYADA Home Health Care is seeking a skilled Physical Therapist to join their team in providing high-quality home health care services.
Responsibilities
- Conduct home visits to clients in designated geographic territories to provide physical therapy services.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions as needed.
- Continuously assess and revise the physical therapy care plan to ensure optimal client outcomes.
- Educate and instruct clients, family members, or other representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided using electronic medical records.
Qualifications
- Current North Carolina Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
