\Responsibilities:
- Make home visits to clients in designated territories.
- Perform diagnostic tests and measurements.
- Develop and implement individualized care plans.
- Educate and instruct clients on rehabilitative care.
- Accurately document observations and interventions.
\Qualifications:
- Current Pennsylvania Physical Therapist license.
- Minimum one year of recent work experience as a Physical Therapist.
- Ability to work independently and manage time effectively.
- Strong interpersonal and organizational skills.
- Solid computer skills and prior EMR experience preferred.
