We are currently looking for an experienced Physical Therapist to join our Hudson County Visits team. The ideal candidate will have one year of recent work experience as a Physical Therapist and a current NJ license.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients in their homes. Your responsibilities will include:

• Making home visits to clients in designated territories.
• Performing diagnostic tests and measurements.
• Implementing integumentary protective interventions.
• Developing and implementing individualized care plans.
• Educating clients and family members on rehabilitative care.
• Accurately documenting client care management and services provided.

Qualifications include:

• A current NJ Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
