Responsibilities:
• Conduct home visits to assess patients' physical abilities and develop customized treatment plans
• Perform diagnostic tests and measurements to identify areas for improvement
• Implement evidence-based interventions to promote wound healing and prevent further injury
• Educate patients and their families on proper care and safety techniques
• Maintain accurate records of patient progress and communicate effectively with the multidisciplinary care team
Requirements:
• Current licensure as a Physical Therapist in the state of New Jersey
• At least one year of experience as a Physical Therapist, preferably in a home health setting
• Strong communication and interpersonal skills, with the ability to work independently and manage time effectively
• Proficiency in electronic medical records (EMR) systems and other relevant software
