We are looking for an experienced Physical Therapist to join our team at BAYADA Home Health, providing home health visits for clients in Salem County, NJ. As a member of our multidisciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients, enabling them to receive the medical care needed to remain at home.
Your responsibilities as a PT - Physical Therapist will include:
- Make home visits to clients in designated territories.
- Perform diagnostic tests and measurements to assess client needs.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Educate and instruct clients, family members, or representatives on rehabilitative care and activities necessary for healthy living.
- Accurately document observations, interventions, and evaluations related to client care management.
To be successful in this role, you will need:
- A current NJ Physical Therapist license.
- At least one year of recent experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, preferably with electronic medical records (EMR) experience.
