About the Role
We are seeking a skilled Physical Therapist to join our team of home health professionals. As a Home Health Physical Therapist, you will provide one-on-one care to adult and geriatric clients in their homes throughout Howard County, Maryland.
Responsibilities:
- Conduct home visits to assess clients' needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Implement interventions to promote wound repair, prevent falls, and improve overall function.
- Collaborate with nursing staff to develop and implement comprehensive care plans.
- Document observations, interventions, and evaluations accurately and timely.
Requirements:
- A current Maryland Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, preferably with electronic medical records (EMR) experience.
