Job Title: Home Health Physical Therapist
Summary:
This role involves performing home health visits throughout Stanly County, NC, providing rehabilitative care and skilled nursing to adult and geriatric clients. As a licensed Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team.
Responsibilities:
- Conduct home visits to assess clients' physical therapy needs.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
- Continually assess and revise the Physical Therapy care plan as needed.
- Document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements:
- Current North Carolina Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
