Job Summary:
We are seeking an experienced Physical Therapist to join our team of compassionate healthcare professionals. As a home health physical therapist, you will work independently with clients in their homes, providing personalized rehabilitation care and skilled nursing services.
Responsibilities:
- Conduct home visits to assess client needs and develop individualized care plans
- Perform diagnostic tests and measurements, including range of motion, strength and endurance assessments
- Develop and implement manual therapeutic exercises, gait training, balance, and other interventions
- Continuously assess and revise care plans as needed, collaborating with nursing and other healthcare professionals
- Educate clients and family members on rehabilitative care and activities necessary for independent living
- Accurately document observations, interventions, and evaluations using electronic medical records
Requirements:
- Current Vermont Physical Therapist license
- Minimum one year of recent experience as a Physical Therapist
- Graduation from an accredited Physical Therapy program
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Computer skills and prior experience with electronic medical records (EMR) preferred
