Home Health Physical Therapist

Job Summary:

We are seeking an experienced Physical Therapist to join our team of compassionate healthcare professionals. As a home health physical therapist, you will work independently with clients in their homes, providing personalized rehabilitation care and skilled nursing services.

Responsibilities:

• Conduct home visits to assess client needs and develop individualized care plans
• Perform diagnostic tests and measurements, including range of motion, strength and endurance assessments
• Develop and implement manual therapeutic exercises, gait training, balance, and other interventions
• Continuously assess and revise care plans as needed, collaborating with nursing and other healthcare professionals
• Educate clients and family members on rehabilitative care and activities necessary for independent living
• Accurately document observations, interventions, and evaluations using electronic medical records

Requirements:

• Current Vermont Physical Therapist license
• Minimum one year of recent experience as a Physical Therapist
• Graduation from an accredited Physical Therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Computer skills and prior experience with electronic medical records (EMR) preferred
