Job Description
This position involves providing ongoing home health visits as a licensed Physical Therapist to clients in Orleans County, VT and surrounding areas.
We are seeking an experienced full-time or part-time Physical Therapist for this opportunity. One year of recent work experience as a Physical Therapist is required.
The ideal candidate will have strong interpersonal, organizational, and problem-solving skills, with solid computer skills and prior experience with electronic medical records (EMR) preferred.
Responsibilities include:
- Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan as appropriate.
- Educating and instructing clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided using a state-of-the-art touch pad tablet.
