Full-Time Physical Therapist - Home Health

We are currently seeking an experienced Full-Time Physical Therapist to provide exceptional care to adult and geriatric patients in their homes in Chatham, Brewster, Orleans and surrounding towns. Prior home care experience is preferred, but not required.
Responsibilities include:
- Making visits to clients in their homes
- Performing diagnostic tests and measurements
- Developing and implementing individualized care plans
- Educating and instructing clients and family members
- Accurately documenting observations and interventions
Qualifications include:
- A current license as a Physical Therapist in Massachusetts
