Job Title: Home Health Physical Therapist
We are seeking a skilled and compassionate Home Health Physical Therapist to join our team in Middle River, Maryland.
About the Role:
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess client mobility and function.
- Develop and implement individualized care plans to promote client health and independence.
Responsibilities:
- Conduct thorough assessments of client needs and create personalized treatment plans.
- Provide manual therapeutic exercises, gait training, and other interventions to improve client mobility and strength.
- Collaborate with nursing staff to ensure comprehensive care and accurate documentation.
Requirements:
- Current MD Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
