Job Title: Home Health Physical Therapist
Summary
We are seeking a highly skilled and compassionate Home Health Physical Therapist to join our team. As a Physical Therapist, you will work with adult and geriatric clients in their homes, providing rehabilitative care and skilled nursing services.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess client's physical abilities.
- Develop and implement individualized care plans to promote client's health and safety.
- Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations related to client care management.
Requirements
- CURRENT MD Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
