Job Description
BAYADA Home Health Care seeks a skilled Physical Therapist for ongoing part-time or per diem home health visits in Frederick, Maryland.
This position offers the opportunity to provide rehabilitative care and skilled nursing services to adult and geriatric clients on a per visit basis. You will be an integral member of a multi-disciplinary healthcare team that aims to enable clients to receive medical care required to remain at home.
Key responsibilities include:
- Conducting home visits to clients in designated geographic territories.
- Performing diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate.
- Educating and instructing clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote their health, safety, and independent living.
- Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided.
To qualify for this role, you must possess:
- A current Maryland Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- The ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) is preferred.
