Job Title: Home Health Physical Therapist
We are seeking a skilled and compassionate Physical Therapist (PT) to join our team in providing high-quality home health care services to seniors in Baldwin, Maryland.
Responsibilities:
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess clients' physical abilities.
- Develop and implement individualized care plans to promote clients' health and independence.
- Educate clients and their families on rehabilitative care and activities necessary for optimal recovery.
- Accurately document client observations, interventions, and evaluations using electronic medical records (EMR).
Requirements:
- A current Maryland Physical Therapist license.
- Graduation from an accredited physical therapy program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with EMR systems.
