Job Summary
BAYADA Home Health Care is seeking a skilled and dedicated Physical Therapist Assistant (PTA) to provide high-quality care to adult and geriatric patients in their homes throughout Washington County, Vermont. As a PTA, you will assist clients in achieving optimal physical well-being by providing treatment, tests, client education, and interventions as delegated by a licensed Physical Therapist.
Key Responsibilities:
- Provide home health visits for adult and geriatric patients in territories throughout Washington County, Vermont.
- Assist clients in attaining or maintaining optimal physical well-being by providing treatment, tests, client education, and appropriate interventions as delegated by a licensed Physical Therapist.
- Collaborate with a team of healthcare professionals to develop and implement comprehensive care plans.
- Conduct assessments and evaluations to identify client needs and goals.
- Develop and implement individualized treatment plans that address client goals and objectives.
- Document client progress and outcomes in accordance with company policies and procedures.
Requirements:
- CURRENT LICENSURE OR CERTIFICATION IN THE STATE OF VERMONT REQUIRED.
- Graduate from a two-year college level program approved by the American Physical Therapy Association.
- OR two years of experience as a Physical Therapist Assistant under the supervision of a qualified Physical Therapist.
- One year's work experience under the supervision of a qualified Physical Therapist.
