The ideal candidate will have experience providing home health visits for adult and geriatric patients on a per visit basis in territories throughout Union County, Somerset County, and/or Middlesex County.
As a Home Health Physical Therapist Assistant (PTA), you will work collaboratively with our licensed Physical Therapists to provide high-quality patient care. Your primary responsibilities will include:
- Assisting clients in attaining or maintaining optimal physical well-being through treatment, testing, client education, and interventions as delegated by a licensed Physical Therapist.
- Providing personalized care plans tailored to each patient's unique needs and goals.
- Maintaining accurate records and documentation of patient progress and outcomes.
- Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
- Current licensure or certification in the state of NJ.
- Graduation from a two-year college-level program approved by the American Physical Therapy Association.
- Two years of appropriate experience as a Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved, or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977.
- At least one year's work experience under the supervision of a qualified Physical Therapist.
