About the Role: As a Rehabilitation Therapist - Adult and Geriatric Care with BAYADA Home Health Care, you will play a vital role in helping patients recover from illness or injury and regain their independence. Your primary responsibility will be to conduct thorough evaluations of patients' functional status, identify equipment needs and environmental adaptations, and develop individualized intervention plans to achieve favorable outcomes. You will also educate patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Key Responsibilities: Make home visits to clients in designated geographic territories, perform thorough evaluations of patients' functional status, assess cognitive, psychological, and motor aspects of performance, and develop individualized intervention plans. Continually assess and revise the occupational therapy care plan, participate with nursing in the multidisciplinary care plan, and accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Requirements: A current Pennsylvania Occupational Therapist license, a minimum of one year of recent work experience as an Occupational Therapist, graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, and strong interpersonal skills, organizational skills, and problem-solving skills. Prior home care experience is preferred, but not required.
