BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team of healthcare professionals in providing rehabilitation services to adult and geriatric patients in Senior Living Communities.
About the Role
- As a Physical Therapist with BAYADA, you will have the opportunity to work closely with clients and their families to develop and implement personalized care plans that address their unique needs and goals.
- You will conduct home visits to assess client needs and develop targeted interventions, such as manual therapeutic exercises and gait training, to promote independence and improve mobility.
Key Responsibilities:
- Conduct comprehensive assessments of clients' physical function and mobility to identify areas for improvement.
- Develop and implement individualized care plans that address client goals and needs.
- Educate clients and their families on safe ambulation techniques and strategies to prevent falls.
- Accurately document observations, interventions, and evaluations to ensure continuity of care.
Requirements:
- A current Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to communicate effectively with clients, families, and healthcare team members.
