BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Gwynedd, Pennsylvania. Main Responsibilities:
- To assess clients' physical capabilities and limitations, identifying areas for improvement and developing targeted interventions to address these needs.
- To design and implement comprehensive physical therapy programs, incorporating modalities such as exercise, education, and equipment recommendations, to optimize client outcomes.
- To communicate effectively with clients, families, and healthcare providers, ensuring seamless coordination of care and ongoing support.
- To maintain accurate and timely documentation of client progress, facilitating informed decision-making and smooth transitions between care settings.
Required Skills and Qualifications:
- A current PA Physical Therapist license and at least one year of experience as a Physical Therapist.
- Graduation from an accredited physical therapy program.
- Excellent interpersonal, organizational, and problem-solving skills, with strong communication and time management abilities.
- Prior experience with electronic medical records (EMR) is highly desirable.
