About the Job
BAYADA Home Health Care is seeking a skilled and compassionate Home Health Speech Language Pathologist to join our team in Big Cove Tannery, Pennsylvania.
The ideal candidate will have experience working with patients who require speech therapy services in their homes, providing thorough evaluations, developing treatment plans, and educating patients and their families on rehabilitative care and activities necessary for independent living.
Responsibilities
- Conduct home visits to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
- Develop and implement effective treatment plans based on evaluation results, physician's orders, and recommended goals.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary for health, safety, and independent living.
- Maintain accurate records of observations, interventions, and evaluations related to patient care management and services provided.
Requirements
- A current license as a Speech Language Pathologist in PA.
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
