Job Description:
BAYADA Home Health Care is seeking an experienced Speech Language Pathologist for ongoing home health visits in Fulton County, PA.
Prior home care experience as a Speech Language Pathologist is a plus, but not required.
Responsibilities:
- Make home visits to clients and perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties.
- Develop a treatment plan based on the evaluation and physician's orders, and continually assess and revise the care plan as necessary.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Qualifications:
- A current license as a Speech Language Pathologist in PA.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
