Job Title: Speech Language Pathologist, SLP - Home Health
About the Job
We are seeking an experienced Speech Language Pathologist to join our home health team in Cranford, NJ. As a Speech Language Pathologist, you will provide rehabilitative care and skilled nursing to clients in their homes throughout Somerset and Middlesex Counties.
Responsibilities:
- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties.
- Develop treatment plans based on evaluations, physician's orders, and recommended goals.
- Continually assess and revise treatment plans and participate with BAYADA clinicians in multidisciplinary care planning as needed.
- Educate and instruct patients, family members, or patient representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to patient care management and services provided.
Requirements:
- A current license as a Speech Language Pathologist in NJ.
- A certificate of clinical competence in speech from the American Speech and Hearing Association or equivalent educational requirements and work experience.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
