BAYADA Home Health Care is seeking a skilled Speech Language Pathologist for a full-time opportunity performing home health visits in Cranford, NJ.
About the Opportunity
- Location: Cranford, NJ
- Clients: Adult and Geriatric patients
- Duties:
Responsibilities:
- Make home visits to clients in designated territories.
- Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties.
- Develop a treatment plan based on evaluation results, physician's orders, and recommended goals.
- Continually assess and revise the care plan, participating in multidisciplinary care planning as needed.
- Educate patients, family members, or representatives on rehabilitative care and activities to promote healthy living.
- Accurately document observations, interventions, and evaluations using state-of-the-art touch pad tablets.
Qualifications:
- A current license as a Speech Language Pathologist in New Jersey.
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association or equivalent qualifications.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
