We are seeking a highly motivated and experienced Speech Language Pathologist to join our team at BAYADA Home Health Care. As a member of our multidisciplinary healthcare team, you will be responsible for providing rehabilitative care and skilled nursing services to patients in their homes.
The ideal candidate for this position will have a current license as a Speech Language Pathologist in NJ, as well as the equivalent educational requirements and work experience necessary for such a certificate. Prior experience with electronic medical records (EMR) is preferred, but not required. Key Responsibilities:
- Conduct thorough evaluations to assess and diagnose clients' speech, language, communication, or swallowing difficulties.
- Develop and implement individualized care plans to address clients' needs and goals.
- Provide education and training to patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Maintain accurate and detailed documentation of patient interactions, assessments, and interventions using electronic medical records (EMR).
Requirements:
- A current license as a Speech Language Pathologist in NJ.
- The equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate.
