We are seeking a skilled Home Health Physical Therapist to join our team in Monett, Missouri. As a member of our team, you will be part of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Job Description
Our Home Health Physical Therapist will deliver exceptional care to every client. Key responsibilities include:

• Making home visits to clients in designated geographic territories
• Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments
• Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions

Requirements
To be successful in this role, you must have:

• A current Physical Therapist license in the state
• A minimum of one year of recent work experience as a Physical Therapist
• Previous home health care experience welcomed, but not required
