Job Description
- Provide high-quality, patient-centered care to clients in their homes, focusing on their unique needs and preferences.
- Assess and develop individualized care plans, incorporating input from healthcare teams and family members.

Requirements and Qualifications
- A current, active RN license in the state of Pennsylvania.
- A minimum of one year of recent, verifiable clinical experience in a hospital or home health setting.
- Prior experience with electronic medical records (EMRs) and other healthcare technologies.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to travel to cases as assigned, with reliable transportation and valid driver's license.
