BAYADA Home Health Care is seeking a skilled Medical Case Manager to join our team in Haddon Heights, New Jersey.

We require an experienced Registered Nurse to perform emergency night visits as needed. This office will service clients on a per visit basis throughout Camden county.
A rotating schedule with on-call hours from 5pm to 8:30am and one week on, the next off
Full Time compensation and benefits, but only work 26 weeks out of the year
Key Responsibilities:
- Manage client care plans and ensure seamless delivery of services
- Collaborate with healthcare teams to develop and implement comprehensive care plans
- Conduct home visits to assess clients' needs and progress
- Document all interactions with clients and healthcare teams accurately and thoroughly
- Maintain a safe and healthy environment for clients
Requirements:
- A current license as a Registered Nurse in NJ
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong communication and interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
