Job Title: Registered Nurse (RN) Home Health Manager, Rehabilitation
We are seeking a skilled registered nurse to join our team as a Rehab Manager in our office, responsible for leading a multi-disciplinary care team in delivering exceptional client service and quality clinical care.
Key Responsibilities:
- Direct and coordinate care provided by a multi-disciplinary care team, ensuring seamless delivery of services.
- Supervise field staff in delivering exceptional client service and quality clinical care, adhering to policies and procedures.
- Monitor Quality Assurance, ensuring compliance with regulatory requirements.
- Participate in ongoing education, evaluation, and development of field staff, promoting continuous improvement.
Requirements:
- Current license in good standing, Physical Therapist (PT), Occupational Therapist (OT), or Speech Language Pathologist (SLP).
- Prior supervisory experience, with a strong understanding of home health care principles.
- Minimum of two (2) years community health experience, with prior clinical and managerial experience within a Medicare Certified Home Health agency.
- Comprehensive knowledge of OASIS and the Medicare Conditions of Participation for Home Health, with strong clinical assessment and case management skills.
