We are seeking an experienced Occupational Therapist to join our home health care team at BAYADA Home Health Care.

The successful candidate will work closely with a multi-disciplinary healthcare team to provide in-home rehabilitative care and skilled nursing services to clients, allowing them to receive the necessary medical attention to remain at home.
Responsibilities:
- Evaluate patients in designated senior living communities or group homes for adults with intellectual disabilities to develop a comprehensive plan of care.
- Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring seamless client-centered goals.
- Educate clients, families, and community caregivers on created care plans to maximize functional independence.
- Perform evaluations, treatments, and proper discharge planning for patients utilizing an outpatient EMR platform.
Qualifications:
- A current NJ Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
