About the Position
We are seeking an experienced Occupational Therapist to join our team in providing high-quality home health care services. As a key member of our multidisciplinary healthcare team, you will be responsible for delivering individualized occupational therapy services to patients in their homes.
Job Responsibilities:
- Evaluate patients' functional status and develop personalized intervention plans to enhance their independence and quality of life.
- Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas for improvement and developing strategies for rehabilitation.
- Create and implement customized occupational therapy plans, incorporating activities that promote patient engagement, motivation, and progress.
- Educate patients, family members, and caregivers on proper techniques and strategies for maintaining a safe and accessible living environment.
- Maintain accurate and detailed records of patient interactions, progress, and treatment outcomes, utilizing electronic medical records (EMR) systems.
Requirements:
- Current licensure as an Occupational Therapist in the state of Maryland.
- At least one year of recent experience in home health care or a related field.
- Prior experience working with geriatric populations is preferred but not required.
- Able to work independently and manage time effectively, prioritizing tasks and adapting to changing circumstances as needed.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
