Responsibilities
- Conduct thorough assessments of patients' physical, cognitive, and emotional status.
- Develop and implement individualized treatment plans to address patients' specific needs and goals.
- Evaluate the home environment for hazards or barriers to independent living and provide recommendations for modifications and adaptations.
- Identify equipment needs and arrange for necessary assistive devices to support patients' rehabilitation and independence.
- Maintain accurate and detailed records of patient interactions, including progress notes, treatment plans, and outcome measurements.
