As a home care Physical Therapist, you will be part of a multi-disciplinary team providing rehabilitative care and skilled nursing services to clients in their homes.
Responsibilities include making outpatient visits, performing diagnostic tests, developing and implementing individualized care plans, educating clients and family members, and accurately documenting client care management.
Qualifications include a current Pennsylvania Physical Therapist license, at least one year of recent work experience as a Physical Therapist, graduation from an accredited program, ability to work independently, strong interpersonal and organizational skills, and solid computer skills.
